Cash-Pay & Good Faith Estimate Notice
NorthStar Integrative Medicine is a direct-pay medical practice. We do not bill insurance for our professional services. Patients are responsible for payment at the time services are provided, unless otherwise arranged in writing.
Under federal law, you have the right to receive a Good Faith Estimate of the expected cost of your care. This estimate will outline the anticipated charges for scheduled services. You may request a Good Faith Estimate before your visit or at any time by contacting our office.
If you receive a bill that is substantially higher than your Good Faith Estimate, you have the right to dispute the charges through the federal patient-provider dispute resolution process.
We will also provide detailed receipts upon request for your records, reimbursement submissions, or personal tracking.
No Emergency Care
NorthStar Integrative Medicine does not provide emergency or urgent care services. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Outcomes & Results Disclaimer
Individual results vary. Hormone therapy, peptide therapy, and other medical treatments do not produce identical results for every patient. Weight changes, symptom improvement, energy levels, and other outcomes depend on many factors including biology, adherence, lifestyle, and underlying health conditions.
