These Terms of Service describe your rights and responsibilities when you use NorthStar Integrative Medicine (“NorthStar”) services, including in-person visits, telehealth services, telephone communications, patient portals, payment systems, and digital platforms.

By scheduling an appointment, using our website or portal, or receiving care, you agree to these Terms.

Your Right to Privacy

Your privacy is a fundamental part of your care. NorthStar is a healthcare provider subject to the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws.

We collect and use only the information necessary to provide medical care, manage your account, and meet legal and regulatory obligations.

This includes:

• Medical history and clinical records
• Lab results and prescriptions
• Payment and billing information
• Appointment records
• Communications with our team

We do not sell or monetize patient data.

Telehealth Rights & Limitations

You may receive care via telehealth. Telehealth has limitations, including the inability to perform a physical exam. Your provider may require in-person visits, lab testing, or referrals when necessary.

You may withdraw consent for telehealth at any time.

Financial Rights & Responsibilities

NorthStar is a direct-pay medical practice. We do not bill insurance for professional services.

You have the right to:
• Know the cost of services
• Request a Good Faith Estimate
• Receive itemized receipts

You are responsible for payment for all services. Labs and medications are billed separately by third parties.

Medical Decision-Making

All treatment decisions are based on medical judgment, clinical guidelines, and your individual health profile. Not all patients are eligible for all treatments. We may decline, modify, or discontinue care if it is medically or legally appropriate.

Prescriptions & Compounded Therapies

Medications, including compounded medications and peptides, may be prescribed based on clinical judgment. Some treatments may be used off-label, which is lawful and common in medical practice.

No medication is guaranteed.

Outcomes & Expectations

Results vary. NorthStar does not guarantee symptom relief, weight changes, hormone levels, or health outcomes.

Records Retention

Medical records are retained in accordance with state and federal law. You may request copies, but records cannot be deleted.

Termination of Services

NorthStar may discontinue care for non-payment, non-compliance, safety concerns, or legal requirements.

Complaints & Concerns

You may submit privacy or care concerns without retaliation. We will review and respond in accordance with law..
